Job summary

UnitedHealth Group's Optum Insight is seeking a Clinical Pharmacist to join the ACO Practice Extend team. You will lead medication therapy management for patients, collaborate with care teammates, and work to improve outcomes while controlling costs.

This is a remote role within the United States. Qualified candidates will have Pharm.D., active licensure, 3+ years MTM, bilingual Spanish/English, and flexibility to work evenings.

Qualifications

• Pharm.D. degree required and eligible for licensure.
• Active pharmacist licensure in the state of residence in good standing.
• Ability to obtain additional state licenses as needed.
• 3+ years of direct MTM and CMR experience in a clinical setting.
• Spanish/English bilingual with strong communication skills.

Responsibilities

• MTM and CMR for high-risk patients and those referred by PCPs.
• Coordinate follow-up care and optimize therapeutic outcomes.
• Engage providers with clear, patient-specific drug recommendations.
• Document MTM interventions accurately in EMRs.
• Collaborate with care team to improve outcomes and reduce costs.
