Job summary

Guidehealth is seeking a highly motivated Clinical Pharmacist (PharmD) to join our remote team. The role focuses on improving HEDIS medication adherence, driving quality measures, and enhancing patient outcomes across care settings.

You will lead MTM sessions, analyze adherence data, collaborate with clinicians, and support population health initiatives through data-driven strategies.

Qualifications

• PharmD degree from an accredited school of pharmacy.
• Active pharmacist license in TX, PA, AL, or TN.
• Minimum of 2 years in HEDIS-related medication adherence, quality improvement, or population health.
• Strong knowledge of HEDIS measures related to medication adherence.
• Proficiency with EHR systems and data analysis tools.
• Excellent communication, problem-solving, and organizational skills.

Responsibilities

• Lead efforts to improve HEDIS medication adherence and other quality measures.
• Analyze medication adherence data, identify gaps, and implement interventions.
• Collaborate with providers, care managers, and stakeholders to promote adherence to therapies.
• Conduct MTM sessions, educate patients, and address barriers to adherence.
• Monitor HEDIS performance metrics and generate reports on interventions.
