Devoted Health is seeking a Pharmacist to join a high-performing team advancing pharmacist-led care. You will collaborate with providers to optimize medication therapy, lead chronic disease education, and guide patients through benefits navigation to improve outcomes.

The role emphasizes direct patient care, MTM, and use of AI-enabled tools, with licensure in multiple states and Spanish fluency as key requirements. Remote work options are available in the US.

Qualifications

• 3+ years of clinical experience in direct patient care.
• Licensure in good standing in multiple states (AL, FL, GA, IN, MO, OH, NC, SC, TX) and ability to obtain licensure in at least 5 states.
• Ability to travel to secure licensure as needed.
• Residence in Central, Mountain, Pacific or Hawaiian time zones or ability to work typical clinic hours in those time zones.
• Fluent proficiency in Spanish.

Responsibilities

• Pharmacist Consult Services – manage chronic conditions with personalized care plans and develop educational materials for local population.
• Medication Therapy Management – conduct comprehensive reviews for patients with complex regimens and resolve therapy issues.
• Quality Programs – drive high performance on pharmacy-related clinical measures and address therapy gaps.
• Practice Operations Support – on-call rotations and act as SME for pharmacy model development; mentor the wider team.
• Utilize AI-enabled clinical tools to support documentation and improve tooling accuracy.
