Job summary

Scene Health seeks a Clinical Pharmacist (PharmD) to support the Patient Solutions team. You will conduct MTM services, Comprehensive Medication Reviews, and collaborate with clinicians to optimize therapy and support adherence.

You will interact with patients via video and phone, document care notes, and help develop operational policies. A Puerto Rico residency, active license, and bilingual skills are required.

Qualifications

• Must reside in Puerto Rico.
• Active Pharmacist License with at least 3 years of pharmacy practice.
• Fluent in both English and Spanish, reading, writing, speaking and understanding.
• Ability to obtain licensure in additional states as needed.
• Excellent telephonic and communication skills to serve elderly population.
• Ability to work independently and within a virtual/decentralized team.

Responsibilities

• Conducts MTM services and Comprehensive Medication Reviews (CMRs).
• Deliver MTM services to optimize medication therapy and clinical outcomes.
• Engage with patients via video consultations and phone for defined services.
• Document clinical recommendations and patient care notes clearly and timely.
• Coordinate with Scene Team (nurses, health coaches, management, product managers).
• Meet productivity and performance expectations and develop/update policies.
