Job summary

Cadence is a clinical AI company delivering virtual chronic care support for Medicare patients enrolled in Cadence care programs. The Clinical Navigator (Medical Assistant) will perform proactive outreach, coaching, and care coordination to close gaps in care and support patients remotely.

The role emphasizes documenting interactions, collaborating with the clinical team, and using AI-assisted tools to inform patient decisions while maintaining clinical judgment.

Qualifications

• Active Medical Assistant certification from an accredited association is required.
• At least 5 years of experience as a Medical Assistant is required.
• Remote work readiness (including stable high-speed WiFi) is required.
• Fluency in Spanish is strongly preferred.
• Fluency with AI-assisted clinical tools and ability to discuss AI support with providers/patients is preferred.

Responsibilities

• Outreach to patients by phone and provide one-on-one coaching for chronic conditions.
• Educate and coach patients on behavior modification, nutrition, physical activity, and self-management.
• Conduct comprehensive assessments of health status, lifestyle, and readiness to change.
• Educate patients on disease management, medication adherence, and prevention strategies.
• Monitor patient progress and health outcomes through check-ins and remote monitoring.
• Document interactions and outcomes in Cadence platform; connect patients with resources as needed.
