Job summary

Cadence is a clinical AI company delivering virtual chronic care for Medicare patients. We seek a Licensed Practical Nurse to provide proactive outreach, coaching, care coordination, and documentation within Cadence platforms, collaborating with clinicians to close care gaps.

The role is fully remote in the United States, with standard M-F hours. Candidates should show strong communication, autonomy, and ability to navigate AI-assisted tools while maintaining clinical judgment.

Qualifications

• Active compact multi-state LPN/LVN license required.
• At least 5 years of clinical nursing experience required.
• Remote work readiness with stable high-speed WiFi.
• Spanish fluency preferred.
• Experience with AI-assisted clinical tools preferred.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching to patients with chronic conditions.
• Help patients execute personalized care plans and coach on behavior modification and self-management.
• Conduct comprehensive assessments of health status, lifestyle, and readiness to change.
• Educate patients on disease management, medication adherence, and prevention strategies.
• Monitor progress, document interactions, and follow up with patients in Cadence platform.
