Job summary

Cadence is hiring a Clinical Navigator (Medical Assistant) for virtual chronic care support for Medicare patients enrolled in Cadence programs. You will perform outreach, coaching, and care coordination, documenting interactions in the Cadence platform and collaborating with clinicians to address barriers to care.

The role requires an active Medical Assistant certification and at least 5 years of MA experience, with remote work readiness and strong communication skills.

Qualifications

• Active Medical Assistant certification from an accredited association.
• At least 5 years of experience as a Medical Assistant.
• Remote work readiness with stable high‑speed WiFi.
• Spanish fluency strongly preferred.
• Experience with AI-assisted clinical tools and discussing AI in care delivery.

Responsibilities

• Conduct outbound patient outreach and provide coaching for chronic conditions.
• Help patients execute personalized care plans with education and behavior change techniques.
• Assess health status, lifestyle, and readiness to change.
• Educate on disease management, adherence, and prevention per policies.
• Monitor progress and outcomes via check-ins and remote tools.
• Document interactions and coordinate with care teams in Cadence Platform.
• Collaborate in real time with clinicians using Cadence Platform, Google Workspace and Slack.
