Job summary

Cadence is hiring a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. The role involves outreach, coaching, care coordination, and documenting interactions in the Cadence platform, with close collaboration to the clinical team to ensure patients feel supported and remain engaged throughout their care journey.

The position is full-time and remote within the United States, with standard hours Monday–Friday

Qualifications

• Active compact multi-state LPN/LVN license required.
• At least 5 years of clinical experience as an LPN.
• Remote work readiness with reliable high-speed WiFi.
• Spanish language skills preferred.

Responsibilities

• Outreach and coaching for patients with chronic conditions.
• Educate and support adherence to personalized care plans.
• Coordinate care and address barriers to care.
• Document interactions and outcomes in the Cadence platform.
• Collaborate with clinicians and support teams in real time.
