Vynca Inc. is seeking a Clinical Intake Specialist to join the ECM care management team. You will engage with clients by phone to understand needs, review medical charts, and connect them to the right resources for coordinated care.

You will conduct 8-12 intake visits daily, document actions in VyncaOS, and help develop goals of care with a person-centered approach. Strong communication and multi-tasking are essential in this fast-paced environment.

Formación

• Must have active California LVN license or related degree/cert.
• Minimum 2 years intake/admissions in a high-volume setting.
• Experience with complex care ECM populations.
• Experience with Health Gorilla or Health Record Systems.
• Bilingual English/Spanish preferred.

Responsabilidades

• Conduct outreach to newly referred clients and log activity in VyncaOS.
• Conduct 8-12 intake visits daily with efficient assessments.
• Assist clients in completing intake including psychosocial assessment and goals of care.
• Review medical charts and document client needs and interventions.
• Communicate critical information to care team for personalized planning.
• Utilize Motivational Interviewing and Trauma-Informed care principles.
• Attend weekly Team Operations Meetings.
• Maintain a structured schedule to optimize intake and engagement.
• Proactively outreach to no-shows or uncontacted clients.
• Remain available for warm transfers from field teams for same-day responses.
• Thrive in a fast-paced environment and meet KPIs.
