Job summary

Vynca seeks a Clinical Intake Specialist to join our ECM care management team. You will work directly with clients to understand their needs and connect them to the correct resource on our care team, using phone-based, person-centered care.

Ideal candidates have 2+ years of intake experience in high-volume settings, CA LVN license or related degree, and strong communication across Health Gorilla and health record systems. This role combines remote and in-person work.

Qualifications

• Has an active, unrestricted CA LVN license or a related degree/certificate.
• Minimum 2 years of intake/admissions experience in a high call volume environment.
• Experience with complex care clients and ECM populations.
• Experience with Health Gorilla or Health Record Systems.
• Strong written and spoken communication; collaborative attitude.

Responsibilities

• Conduct outreach to newly referred clients and log activity in VyncaOS.
• Perform 8–12 intake visits daily with accurate assessments.
• Assist clients with initial intake including psychosocial assessment and goals of care.
• Review medical charts and document client needs and interventions.
• Communicate critical information to the care team for personalized planning.
• Apply evidence-based practices (Motivational Interviewing, Harm Reduction, Trauma-Informed Care).
• Attend weekly Team Operations Meetings.
• Maintain an organized schedule to optimize intake and engagement.
• Proactively reach out to no-shows or uncontacted clients.
• Remain available for warm transfers from field teams and ensure same-day responses.
• Adapt to a fast-paced environment with key performance metrics.
