Job summary

Vynca Inc. is seeking a Clinical Outreach Specialist to engage prospective clients, educate them about Enhanced Care Management services, and guide enrollment. You will assess client needs, document interactions, and support the care team with person-centered planning.

The role requires strong communication, experience with complex care populations, and the ability to work remotely from a quiet workspace. Prior LVN/MA credentials and bilingual skills are highly valued.

Qualifications

• 2+ years of telephonic outreach or enrollment in high-volume environments.
• Experience with complex care clients and ECM populations.
• Excellent oral and written communication skills.
• Remote work with a quiet, secure workspace.
• Willing to work 8:30am-5:00pm PT, Mon-Fri; evenings/weekends as needed.

Responsibilities

• Outreach to prospective clients and document interactions in CRM.
• Convey information to care team for personalized plans.
• Assist with psychosocial assessments and care plan development.
• Review medical charts to identify client needs and interventions.
• Apply Motivational Interviewing, Harm Reduction, Trauma-Informed Care.
• Attend weekly Team Operations Meetings.
