Job summary

AllOne Health is a remote, full-time role seeking a Clinical Case Manager to provide advanced clinical consultation and case management for the Client Relations team. You will manage complex referrals from intake to closure, coordinating with internal clinical resources and external providers to ensure timely, high-quality service delivery.

Qualifications

• Licensed to practice in Illinois
• Experience managing formal referrals or fitness-for-duty evaluations
• Clinical experience in mental health and substance use assessment and treatment
• Ability to work independently and manage competing priorities
• Experience in account management, patient/customer consultations or presentations

Responsibilities

• Serve as a senior clinical resource for complex referrals and high-risk cases.
• Coordinate assessments, treatment referrals, and follow-up services.
• Act as clinical liaison among referrals, clients, evaluators and providers while maintaining confidentiality.
• Monitor progress, identify risks, and escalate concerns to leadership.
• Maintain accurate case documentation in clinical and CRM systems.
