CVS Health is seeking a dedicated social work case manager to coordinate care for members with chronic and psychosocial needs, across the continuum of care. This remote role involves 25-50% travel within Wayne and Macomb Counties, with a 40-hour work week.

Required are a Master’s Degree in Social Work and LMSW licensure, plus minimum medical social work/case management experience. Travel and strong communication skills are essential.

Responsabilidades

• Coordinates care for members with long-term rehabilitation or psychosocial needs to prevent readmissions.
• Performs assessments for Long-Term Care or case/disease management programs.
• Develops individualized case plans to improve outcomes and reduce costs.
• Communicates with providers and care management staff about patient status.
• Identifies opportunities to ensure members receive necessary care per their benefit plan.
• Provides information and education on social issues, mental health, and available services.
• Develops programs to reduce hospital admissions and improve health outcomes.
• Coaches junior colleagues in social work techniques and processes.
• Collaborates with community members and organizations to address community issues.
