Job summary

Gentiva is seeking a Remote Clinical Care Coordinator to support patients in post-acute care navigation. The role will involve telephonic nurse duties, determining appropriate levels of care, and educating patients and families about available services including home health, hospice, and palliative care.

The position focuses on continuity of care during transitions, ensuring compassionate service, and achieving strong admissions conversion.

Qualifications

• Associate degree required; Bachelor’s degree preferred.
• Minimum of 3 years of post-acute care experience.
• Licensed Registered Nurse (preferred) or LPN with active licensure in a compact state.

Responsibilities

• Serve as main point of contact for patients transitioning to post-acute services.
• Make clinical level-of-care determinations using medical data and records.
• Educate patients and families on home health, hospice, palliative care, and Medicare coverage.
• Advocate for patients to maximize admissions and ensure timely responses.
• Coordinate follow-up and track referral outcomes for high-quality care transitions.
• Provide options and recommendations for post-acute care using multiple tools.
