Job summary

Gentiva Hospice is seeking an experienced Registered Nurse to act as an RN Case Manager, guiding patients and families through the hospice journey in home and community settings. You will deliver skilled nursing care, assess needs, and refine care plans in collaboration with an interdisciplinary team.

The ideal candidate has at least 1 year of RN experience in hospice or home health, holds an active RN license, CPR certification, and a willingness to travel to patient homes.

Qualifications

• 1 year RN experience in hospice, home health, or related setting.
• Hospice or palliative care experience preferred.
• Current RN license in state of employment; CPR certification.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across interdisciplinary teams; consult with physicians.
• Document timely clinical records and maintain infection control standards.
• Provide patient/family education on disease, end-of-life care, medications, and nutrition.
