Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care at home or in supported settings. You will perform assessments, implement symptom management, and educate families on end-of-life care with an emphasis on dignity and comfort.

Join a dedicated team, collaborate with physicians and social workers, and ensure accurate documentation and timely updates to care plans. On-call responsibilities may apply in a supportive, patient‑centered environment.

Qualifications

• Active RN license in the state of employment.
• CPR Certification.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or by telehealth.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update plans of care including visit frequency and risk ID.
• Coordinate care across interdisciplinary teams and with physicians.
