Job summary

Pyramid Consulting, Inc. is seeking a Clinical - Care Navigator for a 06+ months contract, mostly remote within the U.S. You will manage a caseload, assess needs, and connect members with community resources to support access to care.

Required experience includes 2–4 years in care coordination or related field and a relevant degree or clinical certification. Travel for local visits may be needed.

Immediate need for a talented Clinical - Care Navigator. This is a 06+months contract opportunity with long-term potential and is located in U.S(Remote).

Pay Range: $25.60 /hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Manage an assigned caseload of members and conduct outreach to understand their needs and barriers to accessing care.
• Assess social, healthcare, and resource needs, connecting members with appropriate community-based programs.
• Support the development and implementation of individualized care and service plans in collaboration with care managers.
• Coordinate referrals and follow up on services to ensure members can access necessary resources.
• Conduct in-person member visits and local field visits as required.
• Build and maintain relationships with community organizations, healthcare providers, and local service agencies.
• Attend community events and partnership meetings to identify additional resources and expand the community support network.
• Maintain accurate and timely documentation of member interactions, assessments, referrals, and follow-up activities.
• Collaborate with internal care management teams to address complex member needs and escalated when appropriate.

Key Requirements and Technology Experience:

• Degree or relevant background in social work, healthcare, human services, psychology, or a related field.
• Relevant professional or clinical certification.
• 2–4 years of experience in care coordination, case management, member outreach, community resource navigation, or social services.
• Bachelor's degree in a relevant field.
• Valid driver's license and ability to travel locally for in-person visits.
• Ability to work independently and manage field-based responsibilities.
