Job summary

Jaan Health/Phamily is hiring a Clinical Care Lead RN to support patients in a text-based APCM program. You will deliver proactive care, coordinate digital communications, and guide a care team to improve health outcomes in a virtual setting.

The role prioritizes patient engagement, critical thinking, and effective care coordination across CMAs, LPNs/RNs, and social workers, with remote work options and travel as needed.

Qualifications

• Active, unrestricted compact RN license.
• 4+ years in care management or clinical care coordination.
• Experience managing a team.
• Strong critical thinking in a fast-paced environment.
• Ability to manage a digital patient panel and prioritize escalations.
• Excellent written communication and documentation skills.
• Proficient with EMRs and care coordination platforms.
• Reliable internet and private work environment.
• Travel up to 10% possible.

Responsibilities

• Monitor and respond to patient messages and check-ins promptly and compassionately.
• Identify, assess, and triage clinical concerns, SDOH needs, and care barriers.
• Develop and update personalized care plans and educate patients to adhere to plans.
• Oversee clinical quality of the team and train staff for better outcomes.
• Develop and revise care coordination workflows to improve efficiency.
• Lead quality improvement initiatives and conduct case reviews.
• Escalate critical issues to leadership for system-wide improvements.
• Collaborate with CMAs, LPNs/RNs, NPs, and Social Workers for timely escalation.
