Job summary

Cadence is hiring an APCM Supervisor to lead a remote team of up to 20 Registered Nurses delivering Advanced Primary Care Management to older adults with chronic conditions. You own the daily workflow, clinical quality, and operational performance, balancing documentation and audits with coaching and productivity accountability.

Reporting to the APCM/CCM Manager, you will drive team scheduling, hiring, and performance improvements while ensuring adherence to APCM protocols.

Qualifications

• Active, unencumbered RN license (compact preferred).
• Minimum of 5 years RN practice, ideally in Advanced Primary Care Management or chronic care.
• Minimum 1 year supervisory/management experience in quality assurance and performance management.
• Experience overseeing a high-volume, metric-driven remote team.
• Experience leading a remote clinical workforce.
• Fluency with AI-assisted clinical tools (Gemini, Open Evidence) and ability to discuss AI-supported care with providers/patients.

Responsibilities

• Lead day-to-day workflow of a remote team of up to 20 RNs, ensuring productivity and operational metrics for APCM program performance.
• Audit RN performance and documentation for compliance and quality.
• Coach RNs through structured feedback and performance data.
• Manage direct reports with regular 1:1s and improvement plans.
• Collaborate with Training Supervisor to close education gaps.
• Manage scheduling, PTO, and coverage for remote team.
• Lead RN hiring and candidate selection.
