Job summary

Cadence, a clinical AI company, seeks an APCM Supervisor to lead a remote team of up to 20 RNs delivering Advanced Primary Care Management to older adults with chronic conditions. You own daily workflow, clinical quality, and operational performance, balancing documentation and audits with coaching and productivity accountability.

Reporting to the APCM/CCM Manager, you keep clinical standards and program metrics aligned as Cadence extends primary care into patients' homes between visits.

Qualifications

• Active, unencumbered RN license (compact license preferred).
• Minimum 5 years of RN practice, ideally in APCM or chronic care management.
• Minimum 1 year of supervisory/management experience with clinical quality assurance.

Responsibilities

• Lead day-to-day workflow of a remote team of up to 20 RNs, ensuring productivity and metrics.
• Audit RN performance and documentation for quality and protocol adherence.
• Coach RNs through feedback, translating audits into improvement plans.
• Manage direct reports via 1:1s and performance improvement plans.
• Collaborate with Training Supervisor to close education gaps.
• Oversee scheduling, PTO, and coverage to meet demand.
• Drive RN hiring through interviews and candidate selection.
