Qualifications

• Licensed Nurse Practitioner in NJ (ANCC or equivalent).
• 2+ years NP experience with complex adult populations.
• Eligible for Medicare participation; company bills on your behalf.

Responsibilities

• Direct patient care across SNF and Home Health AI Care programs.
• Train and coach facility staff to integrate Circadia into workflows.
• Interpret trend-based insights and drive timely interventions.
• Complete documentation within the same day.
