Job summary

Medical Home Network is seeking a Clinical Services Coordinator to support patients, providers, and care teams. You will engage patients across AWVs, TOC, preventive care, and value-based initiatives using EMR tools and AI-enabled workflows.

The role emphasizes coordinating services across care settings, improving access, and ensuring timely follow-up while leveraging technology and population health reports to enhance the patient experience.

Qualifications

• 3 years of work experience in care coordination or care/case management, patient outreach, and connecting to community resources
• Bilingual in Spanish and fluent in medical terminology
• Experience in safety-net or public health hospitals or Federally Qualified Health Centers (FQHCs) advantageous

Responsibilities

• Conduct proactive patient outreach to support AWVs, TOC, preventive screenings, and care management.
• Schedule, coordinate, and confirm patient appointments while addressing barriers to care.
• Coordinate services across primary care, specialty care, behavioral health, and community partners.
• Build collaborative relationships with patients, caregivers, providers, and community partners to improve access and outcomes.
• Provide patient education, reinforce care plans, and support self-management activities.
