Job summary

Southcoast Health System, Inc. is seeking a Team Lead-Centralized Clinic Support for a remote position that must reside in Massachusetts or Rhode Island.

The role is a 40-hour day shift with every other weekend, holiday, and on-call rotation (7:30am - 4:00pm). The ideal candidate will have at least five years of nursing or clinical experience, ambulatory care nursing experience, and an RN license in MA/RI with dual licensure within 3 months.

Qualifications

• Must have five years of nursing or relevant clinical experience.
• Three years of ambulatory care nursing/medical office experience preferred.
• Bachelor's degree in Nursing or related field highly desirable.
• Supervisory experience preferred.
• Must possess excellent interpersonal, communication and leadership skills and handle multiple priorities.
• Current RN licensure in MA and/or RI; dual licensure required within 3 months of hire.
• Certification in Ambulatory Nursing, Care Management or Care Coordination desirable.
• Current BLS certification required.

Responsibilities

• Coordinate clinical support and monitor personnel productivity.
• Provide training, guidance, and support to nursing and clinical staff to ensure compliance with workflows and policies.
• Monitor daily performance reports to measure staff performance and identify improvements.
• Collaborate to build and maintain daily staffing schedules for Operations.
• Perform ambulatory nursing and centralized services to support patient care and population health management.
• Implement and monitor clinical workflows and address quality and patient safety concerns.
• Onboard and assess competency of clinical staff with Professional Development.
• Collaborate with practice leadership to ensure safe, high-quality patient care.
