Job summary

Evergreen Nephrology is seeking a Program Manager for Chronic and Complex Care to lead a remote CCPM team focused on longitudinal, patient-centered management of kidney disease and comorbidities. You’ll drive proactive care strategies, guide high-risk populations, and ensure adherence to care pathways while reducing complications and hospital use.

You will coach staff, implement performance metrics, and collaborate with market leaders to improve outcomes.

Qualifications

• Bachelor’s degree in healthcare or nursing (BSN required).
• 3+ years of longitudinal clinical nursing care management experience.
• 2+ years in a clinical nursing managerial or leadership role.
• Experience in longitudinal care management and value-based healthcare operations.
• Ability to develop talent, delegate, innovate, and execute with a team.
• Strong communication, relationship-building, and change-management skills.

Responsibilities

• Oversee day-to-day CCPM care management operations with standardized processes.
• Ensure high-quality, longitudinal chronic/complex care management per clinical pathways.
• Evaluate program performance and coach team members for continuous improvement.
• Develop collaborative strategies to improve patient outcomes and reduce readmissions.
• Onboard and train new CCPM team members; provide ongoing coaching.
• Prepare performance reports, dashboards, and materials for internal/external audiences.
• Build relationships with stakeholders to drive outcomes through collaboration.
