Job summary

Resources for Human Development is seeking a dedicated Case Manager to coordinate care, maintain treatment plans, and support youth through a collaborative, treatment-focused approach. You will work as part of a multidisciplinary team, ensuring services align with Individual Service Plans and community resources.

The role requires a Bachelor’s degree with 1 year of experience or equivalent, strong organizational skills, and a compassionate, professional approach.

Qualifications

• Bachelor’s degree and one year of experience preferred OR equivalent combination of education and experience.
• Intermediate computer skills.
• Must have a HIPAA compliant home office setup.
• Must pass drug screening and state and federal background checks.

Responsibilities

• Assist in development of service plans and ISP reviews with participant and team.
• Develop and maintain relationships with community agencies to connect participants to resources.
• Facilitate initial and annual ISP development and reviews.
• Monitor ISP and document outcomes; intervene as needed.
• Review and report Critical Incident Reports per protocol.
• Participate in Human Rights Committee and Behavioral Intervention Committee reviews.
• Focus on Personal Outcome measures and Life Course framework.
