Job summary

Cambia Health Solutions is seeking a Remote Care Management Nurse to join our Clinical Services team. In this role you will provide clinical care management, including case management and care coordination, to support members’ health needs while working from home across Oregon, Washington, Idaho, or Utah.

You will collaborate with physicians and care partners, document care plans, and ensure compliance with medical policies.

Qualifications

• Registered Nurse with at least an Associate's or Bachelor's degree in nursing or health/human services.
• Active, unrestricted RN license or equivalent allowing independent assessments.
• Experience in case management, utilization management, or disease management preferred.
• Eligibility for licensure in target states (OR, WA, ID, UT).

Responsibilities

• Conduct case management activities: assessment, planning, coordination, monitoring and evaluation.
• Apply clinical judgment to ensure medical policy compliance and appropriate care.
• Collaborate with physicians, teams and external partners to resolve issues.
• Serve as a resource while protecting patient confidentiality.
• Document care plans accurately and meet timelines and standards.
• Coordinate resources and prioritize assignments to meet goals.
• Monitor and adjust plans based on effectiveness and new information.
