Qualifications

• Master's degree in Social Work or equivalent license in Oregon required.
• 2+ years in Medicaid-funded case management or care coordination.
• Experience with diverse populations and marginalized communities.
• Training in health disparities and equitable health outcomes.
• Cultural competencies and ability to provide culturally appropriate care.
• Experience communicating with non-English speakers or sign language users.
• Experience engaging underserved communities in health care planning.

Responsibilities

• Assess, plan, implement, monitor, and evaluate individualized care plans.
• Coordinate with healthcare team to ensure optimal client outcomes.
• Maintain client confidentiality and comply with HIPAA rules.
• Manage caseload and ensure documentation in the web-based database.
• Pursue ongoing education and professional development.
• Collaborate with beneficiaries, caregivers, and providers for coordinated care.
