Job summary

CVS Health is hiring an Integrated Care Management (ICM) Case Manager to develop proactive care plans for members with Medicare and Medicaid needs. The role requires traveling 25-50% within NJ, coordinating care with providers and community resources, and conducting comprehensive assessments to improve overall wellness.

Qualified nurses with NJ licensure and 3+ years of clinical practice are encouraged to apply; remote work options are available with expectations for in-person visits as needed.

Qualifications

• Minimum 3+ years of clinical practice experience.
• Active and unrestricted RN licensure in the state of NJ.
• Willing and able to travel 25-50% of time with own vehicle; mileage reimbursed.
• Must reside close to or within Cumberland/Atlantic County, New Jersey.

Responsibilities

• Conduct evaluations of member needs and benefits.
• Apply clinical judgment to reduce risk factors and barriers.
• Assess information from claims and other sources to address conditions.
• Refer to clinical resources and coordinate with interdisciplinary team.
• Collaborate with stakeholders to meet goals; present cases in conferences.
• Follow case management processes in compliance with policies; use motivational interviewing.
