Qualifications

• Associate's degree or nursing diploma required.
• Active and Unrestricted RN license in NJ required.
• Successful completion of the NJ Choice certification is a contingency of employment.
• 3+ years of clinical practice experience in a hospital or alternative care setting.

Responsibilities

• Conduct face-to-face visits in members' homes using comprehensive assessment tools.
• Evaluate the medical needs of members enrolled in MLTSS and D-SNP/FIDE.
• Facilitate requests for services such as adult medical daycare and personal care assistant.
• Schedule and attend interdisciplinary meetings advocating for members' safe discharge.
• Collaborate with members and care teams to develop care plans and authorize services.
• Document accurately and timely in the member's electronic health record.
• Coordinate care with members, authorized representatives, and care team participants.
• Use critical thinking skills to solve problems.
• Mentor new hires.

About This Role

The Case Manager Registered Nurse (RN) will manage a member caseload throughout South and Central New Jersey, conducting in-person patient assessments and coordinating multidisciplinary care plans. This role is ideal for an autonomous and compassionate nurse who thrives in a field-based environment.

Highlights

• Pay: $66,575 - $142,576/year
• Location: Remote, US
• Residency: Must reside in Camden County or the immediate surrounding area
• Schedule: Full-time
• Travel: Approximately 75% travel

What You'll Do

• Conduct face-to-face visits in members' homes using comprehensive assessment tools.
• Evaluate the medical needs of members enrolled in Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Program (D-SNP/FIDE).
• Facilitate requests for services such as adult medical daycare and personal care assistant.

• Collaborate with members and care teams to develop care plans and authorize services.
• Document accurately and timely in the member's electronic health record.

Requirements

Education:
