Job summary

CVS Health is hiring a Case Manager Registered Nurse to deliver comprehensive care management for members with complex medical and behavioral health needs. This remote, work-at-home role is based in New York with a Monday–Friday schedule and requires clinical assessments and care plan development.

You will coordinate with interdisciplinary teams, caregivers and providers, ensure medically necessary services, and support transitions of care while upholding HIPAA and regulatory standards.

Qualifications

• Active RN license in the applicable state.
• 3–5 years of clinical nursing experience, preferably in care management or related settings.
• Experience coordinating care for members with complex medical, behavioral health, and psychosocial needs.
• Strong knowledge of chronic disease management and care coordination.
• Excellent communication and organizational skills.

Responsibilities

• Provide comprehensive care management services to members with complex medical, behavioral health, and social needs.
• Develop and monitor person-centered care plans.
• Coordinate care across providers and community resources.
