The position is work-from-home in New York, with a 40-hour work week, standard 8–5 hours, and a comprehensive benefits package. BSN preferred and RN license required.

Qualifications

• Active and unrestricted RN license in the applicable state.
• 3–5 years of clinical nursing experience, preferably in care management or related settings.
• Experience coordinating care for members with complex needs.
• Knowledge of chronic disease management and care coordination.
• Ability to conduct assessments and develop care plans.
• Excellent communication, organizational, and problem-solving skills.
• Proficiency with MS Office and electronic documentation systems.
• Ability to work independently while collaborating with teams.

• Provide comprehensive care management services to members with complex needs.
• Conduct clinical assessments and develop person-centered care plans.
