Job summary

The Workforce Group, a LEMOINE company, seeks experienced Case Managers to support the ACF OHSEPR program. You will manage intake, assessments, and service coordination for repatriates, ensuring safe reception and transitions to housing and essential services.

Travel may be required; travel arrangements and remote work are possible as the program operates with federal partners. Strong documentation, HIPAA compliance, and experience with vulnerable populations are essential.

Qualifications

Bachelor’s degree in Social Work, Psychology, Human Services or related field.
At least 3 years of case management experience in human services.
Experience with ACF/OHSEPR, emergency response, refugee resettlement or repatriation programs preferred.
Experience with direct client services and needs assessments.
Knowledge of social service resources and referral processes.
Familiarity with TANF, SNAP, Medicaid, and Vocational Rehabilitation Services preferred.
Strong interviewing, documentation, and case reporting skills.
Ability to work effectively with diverse and vulnerable populations.
Proficiency with case management databases (e.g., HMIS) or ability to learn within 30 days.
MS Word, Excel, PowerPoint, and Teams proficiency.
Bilingual or multilingual skills (Spanish and other languages) preferred.

Responsibilities

Provide end-to-end case management including intake, needs assessment, service coordination, and case closure.
Meet repatriates at ports of entry or final destinations, ensuring safe reception and housing or other support services.
Coordinate with federal, state, and local partners and providers to connect resources.
Support complex cases involving vulnerable populations.
Document case management activities, assessments, and referrals in OHSEPR’s system within timelines.
Monitor progress during the 90-day temporary assistance period.
Prepare case notes, reports, and correspondence compliant with program requirements.
Participate in ongoing training, program improvement initiatives, and quality assurance activities.
Adhere to confidentiality, HIPAA, and federal data security standards.
Perform other related duties as assigned.
