Job summary

Humana is seeking a compassionate, organized case manager to join our team serving Wisconsin Family Care members in Weston. The role provides comprehensive case management, including MCP development, coordination with Field Care Nurse, and in-home assessments, with travel up to 40%.

You will work under the Manager of Care Coaching, perform quarterly visits, maintain documentation, and ensure cost-effective services while supporting frail elders and adults with disabilities in a patient-facing,

Qualifications

• Bachelor’s degree in human services or related field required with 1 year experience serving frail elders or related disabilities; or a bachelor’s in another field with 3 years of such experience.
• Valid state driver’s license and proof of personal vehicle liability insurance with 100/300/100 limits

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs in the MCP framework.
• Conduct face-to-face social assessments at enrollment and at least every six months, typically at the member’s residence.
• Conduct quarterly in-person visits and maintain monthly phone contact.
• Arrange support services including social integration, housing, employment, and non-medical needs.
• Ensure cost-effective service delivery.
• Evaluate risk factors and provide education to members.
• Maintain accurate documentation including case notes, service authorizations, and MCP updates.
