Job summary

Fraser, Minnesota’s nonprofit leader in autism, mental health, and disability services, is hiring Case Managers to support CADI/BI and DD waiver clients in the Twin Cities area. You will coordinate care, advocate for access to community resources, and maintain strong documentation to support client success.

Requirements include a bachelor’s degree in Social Work or related field and one year of relevant experience; valid Minnesota license and ability to pass background checks.

Qualifications

• Bachelor’s degree in Social Work or related field; licensure if degree is in Social Work.
• One year of experience in education or treatment of persons with developmental disabilities (QDDP qualified).
• Completion of at least one course focused on developmental disabilities (Fraser can provide this course).
• Valid Minnesota Driver’s License with acceptable record.
• Ability to pass a DHS background study.
• Commitment to promoting diversity, multiculturalism, and inclusion with culturally responsive practice.

Responsibilities

• Providing care coordination, monitoring client health and safety needs, and ensuring services meet client goals.
• Intervening as needed to support clients and resolve service gaps.
• Serving as a consistent advocate and connection to resources.
• Caseloads: CADI/BI and DD Waiver case management with ~35–43 clients per caseload.
