Job summary

Keystone Community Services in St Paul, MN seeks a Disability Services Waiver Case Manager to provide person-centered assessment, planning, facilitation and advocacy for the CADI Waiver program in Ramsey County.

This Full Time exempt position offers a hybrid work model with three office days per week, with Wednesdays in office; the remainder of the week is determined by the candidate and hiring team. Salary range is $47,000 to $51,000 annually, plus a $1,000 hiring bonus after 30 and 90 days.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Experience working with seniors or disabled adults in a community-based setting.
• Strong communication skills and ability to document clearly.
• Ability to manage multiple demands and work independently with supervision.
• Valid Minnesota driver’s license and insurance for travel within Ramsey County.

Responsibilities

• Provide person-centered case management services to locate and access waiver and state plan services.
• Develop and implement a Coordinated Services and Support Plan (CSSP).
• Evaluate and monitor services and progress toward goals.
• Coordinate services via Service Agreement and inform county for authorization.
• Maintain records and accurate case notes for continuity of service.
