Job summary

Fraser is seeking Case Managers to support individuals on CADI/BI & DD waivers in Minnesota. You’ll advocate for clients, connect them to resources, and maintain detailed case notes. Roles involve managing caseloads of adults and some children with regular client meetings.

Fully remote after onboarding, candidates must reside in Minnesota/Twin Cities metro area. Travel to meet clients is required, with 2–3 meetings weekly. Starting pay is $50,000–$52,000 plus a $1,500 signing bonus.

Qualifications

• Bachelor's degree in social work, special education, psychology, nursing, human services or related fields.
• If degree is in social work, social work licensure is required.
• One year of experience with DD populations or contingent on Relias I/DD course (paid by Fraser).
• Experience or interest in working with adults with serious mental illness.
• Commitment to promoting diversity, multiculturalism, and inclusion with culturally responsive practice.

Responsibilities

• Provide client-centered case management for CADI/BI & DD waivers.
• Advocate for Fraser clients and connect them to community resources.
• Maintain organized documentation and case notes.
• Work with a mix of adults and children with varied caseloads (35–43 clients).
