Job summary

Humana is recruiting a Case Manager to serve Wisconsin Family Care members in Mauston and surrounding areas. You will perform comprehensive case management, coordinate with a Field Care Nurse, conduct regular in-home assessments, and arrange social and non-medical supports to improve member well-being.

Travel up to 40% within Juneau County and surrounding areas is required. Monday–Friday, 8:00 am–4:30 pm CST, with pay between 53,700 and 72,600 USD per year.

Qualifications

• Bachelor’s degree in human services or related field is required; 1 year of experience with frail elders or adults with disabilities, or 3 years with disabilities in lieu of degree.
• Valid driver’s license and personal vehicle liability insurance with 100/300/100 limits.
• Live within 45 mins or less of assigned coverage area.

Responsibilities

• Assess members’ health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address needs in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months, typically at the member’s residence.
• Conduct quarterly in-person visits and maintain monthly contact by phone.
• Arrange non-medical support services including social integration, housing, and community resources.
• Maintain accurate documentation including case notes and MCP updates.
