Job summary

The Columbus Organization, LLC is seeking a Part Time Case Manager for remote care coordination in Georgia. The role focuses on coordinating care for adults with significant disabilities, developing Plans of Care, and collaborating with families, providers, and community partners.

You will monitor services, ensure compliance, document activities, and participate in trainings. Requirements include five years of healthcare service delivery or case management and RN license or BA in a related field.

Qualifications

• Five years of healthcare service delivery or case management experience.
• Experience serving severely disabled adults and/or adults with brain injuries.
• RN license or BA/BS in healthcare or related field.
• Proficiency with electronic documentation systems.
• Strong communication and teamwork skills.

Responsibilities

• Develop, coordinate, and implement Plans of Care and Service Plans per ICWP.
• Collaborate with participants, families, providers, and resources to reflect goals.
• Monitor delivery of authorized services as outlined in the Plan of Care.
• Evaluate effectiveness and adjust plans to improve outcomes.
• Ensure regulatory compliance and thorough documentation.
• Maintain relationships with internal and external stakeholders.
• Participate in ongoing training and quality improvement.
• Conduct routine site visits to monitor health and safety.
