Job summary

The Columbus Organization is seeking a Care Coordination Specialist to support individuals with intellectual and developmental disabilities across Georgia. This role requires frequent travel to client sites and coordinating services within the ISP framework.

The ideal candidate has a bachelor’s degree in a human service field and at least two years’ experience in DD/IDD settings, plus strong communication and data-management skills. Remote work is possible when not visiting clients.

Qualifications

• Bachelor’s degree in human service field required.
• Two years of experience with DD/IDD or related disabilities and two years in healthcare/rehabilitative settings.
• Ability to travel regularly within assigned territory and to work remotely when not visiting clients.

Responsibilities

• Assist individuals with intellectual/developmental disabilities to access and coordinate services.
• Coordinate and gather information for support plans.
• Arrange and obtain clinically pertinent assessments.
• Review information for ISP meetings and interventions.
• Help clients identify and access cost-effective services.
• Maintain Medicaid eligibility documentation and monthly contacts.
