Job summary

The Columbus Organization seeks a Mobile Case Manager to assist individuals with intellectual and developmental disabilities in coordinating needed support and services in community settings across Georgia. The role emphasizes data gathering, ISP planning, and Medicaid eligibility support.

You will travel regularly to assigned counties and engage families, clinicians, and service providers to optimize outcomes. Experience in DD/ID and healthcare settings is essential for success.

Qualifications

• Bachelor's degree in human services or closely related field.
• Minimum two years of experience serving persons with DD/IDD and two years in a healthcare/rehabilitative setting.
• Mobile worker with regular travel to multiple work locations.
• Reside in the same territory they are assigned to work in; exceptions may be considered.
• Occasional evening and weekend work may be required.

Responsibilities

• Assists people with intellectual challenges or developmental disabilities to access and coordinate needed support and services in community settings.
• Coordinates and facilitates all activities related to gathering information to be included in the support plan.
• Arranges and obtains clinically pertinent assessments.
• Reviews clinical information and recommendations for interventions and services prior to the ISP meeting.
• Assists the person, family or advocate to identify and access cost-effective services and community support.
• Assists individuals and families in applying for/maintaining Medicaid Eligibility.
• Monitors Medicaid redetermination dates and assists with required paperwork.
• Maintains documentation regarding eligibility in charts.
• Checks the Georgia Health Partnership site monthly for eligibility issues.
• Oversees participation in ISP services and activities.
• Determines if support meets needs and expectations of individual and family.
• Visits individuals at residences or guardians’ locations as needed.
• Schedules ISP meetings respecting family availability.
• Ensures annual plan review/update.
• Maintains monthly face-to-face contact per DBHDD guidelines.
• Documents all contacts/services in monitoring tools.
• Maintains confidentiality and HIPAA compliance.
