Job summary

The Columbus Organization is seeking a mobile case manager to help individuals with intellectual challenges access and coordinate needed services in community settings. You will gather information for support plans, arrange assessments, and review clinical data for ISP meetings while maintaining HIPAA compliance.

Responsibilities include coordinating services, monitoring Medicaid eligibility, and ensuring timely documentation and follow-up within the assigned Georgia counties.

Qualifications

• Bachelor’s degree in human services or closely related field.
• At least 1 year of experience with developmentally disabled populations in case management or coordination.
• Mobile worker requiring regular travel within assigned territory.

Responsibilities

• Assist individuals with disabilities to access and coordinate needed support and services in community settings.
• Coordinate information gathering for the support plan and arrange assessments.
• Review clinical information and recommendations prior to ISP meetings.
• Identify and access cost-effective services and community resources.
• Maintain documentation and ensure HIPAA compliance.
• Ensure monthly face-to-face contact and timely monitoring per agency guidelines.
