Job summary

Oscar Health is seeking a CCM Case Management RN for a remote role available to candidates in Arizona, Florida, Georgia, and Texas. You will educate members, coordinate care across settings, and support transitions from care while partnering with the Clinical Concierge Specialist.

The role reports to the Manager, Clinical Programs, offering base pay $32.60–$42.79 per hour plus benefits and 15 days of vacation per year. Remote work with occasional travel may be required.

Qualifications

• Active, unrestricted RN license in the state of hire.
• Minimum 2 years of clinical experience.
• Telephonic case management experience is a plus.

Responsibilities

• Educate members and coordinate care across ER, inpatient, post-acute, outpatient, and home care settings.
• Assist with transitions of care and discharge planning while maintaining confidentiality and professionalism.
• Conduct comprehensive biopsychosocial assessments to identify member needs and develop outcome-based plans.
• Collaborate with the Clinical Concierge Specialist and other Oscar teams to ensure high-quality care.
• Communicate with members via phone, secure messaging, and in person to provide education on health conditions, medications, and procedures.
