Job summary

GlobalHealth is seeking a Nurse Case Manager to perform telephonic care management for high-risk members, coordinating with providers and the care team to improve long-term wellness. This remote role supports members across states and emphasizes advocacy, education, and coordination of services.

The position requires an Oklahoma RN license with potential preference for a multi-state license and prior managed care experience.

Qualifications

• Active Registered Nurse license in the state of Oklahoma required.
• Active multi-state RN license is preferred.
• Previous experience in managed care/utilization management preferred.

Responsibilities

• Conduct telephonic case management for high-risk members and coordinate care.
• Coordinate with providers to ensure holistic, beneficial outcomes.
• Communicate with members and caregivers respecting culture, language, and needs.
• Provide patient advocacy, support, and crisis intervention as needed.
• Coordinate services including community resources and collaboration with care team.
• Educate members and caregivers on conditions and self-management techniques.
• Participate in SNP MOC elements including HRA, ICP, and ICT.
• Perform TOC activities including post-discharge assessments and medication reconciliation.
