Job summary

Oscar Health is seeking a Case Management RN to join our Clinical Concierge Team in a remote capacity for eligible states. You will educate members, coordinate care across settings, and support discharge planning under the Case Management Supervisor.

Qualifications

• Active RN license issued in the United States.
• 2+ years of clinical experience including hospital, outpatient, or community-based care management.
• 1+ years of experience in care coordination and navigation.

Responsibilities

• Coordinate care across inpatient, outpatient, post-acute, ER, and home care settings.
• Develop care plans and reach out to members undergoing difficult health challenges.
• Coordinate discharge planning with hospital case managers and care teams.
• Educate members by phone or secure messaging on health conditions, medications and procedures.
• Ensure compliance with applicable laws and regulations.
• Support pilot programs and process improvement initiatives.
