Job summary

CVS Health Corporation seeks a Case Management Coordinator to support members by scheduling appointments, assisting with benefits access, and guiding education through care management tools. This full-time telework role requires significant travel (50-75%) across Northwest Illinois counties including Winnebago, Boone, Stephenson, Ogle and Jo Daviess.

You will collaborate with care teams to drive appropriate healthcare outcomes.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or appropriate related field equivalent to program focus.
• Must be willing and able to travel 50-75% of the time in Winnebago, Boone, Stephenson, Ogle and Jo Daviess Counties, Illinois.
• Must have 2+ years of experience of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluation of Members: assess needs/eligibility and recommend care plan.
• Identify high risk factors and appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Collaborate with clinicians to overcome barriers to care.
• Escalate quality of care issues per policy and process.
• Negotiate options to meet member benefits and healthcare needs.
• Coach members for informed medical and lifestyle decisions.
• Participate in team meetings and peer mentoring.
• Support member engagement with providers in decision-making.
• Document care activities per regulatory guidelines.
