Qualifications

• 2+ years in Behavioral Health, Social Services or related field.
• Must travel 50-75% in Will, Kankakee and Grundy Counties, IL.
• Experience with electronic documentation and MS Office.

Responsibilities

• Evaluate members’ needs and eligibility; suggest case resolution approach.
• Identify high risk factors and refer to appropriate care management.
• Coordinate and monitor care plan activities and progress.
• Consult with case managers and medical staff to overcome barriers.
• Escalate quality of care issues through established channels.
• Negotiate options and services to meet member needs.
• Use motivational interviewing to engage members and promote healthy choices.
• Coach and support members to participate in healthcare decisions.
• Participate in team meetings and provide peer mentoring.
• Assist members in provider engagement and decision-making.
• Document care in line with regulatory and company policies.
