Job summary

The Columbus Organization is seeking a dedicated Case Manager in Georgia to assist individuals with intellectual and developmental disabilities. You will help access and coordinate needed supports in community settings, building strong relationships with clients and families.

Knowledge of Medicaid processes, ISP planning, and data collection is important; you will ensure eligibility, document accurately, and liaise with clinicians and agencies to meet client goals.

Qualifications

• Must have a bachelor’s degree in a human service field, such as social work, psychology, special education, nursing, or a closely related field.
• At least one year of experience serving persons with developmental disabilities in case management or coordination; Master’s degree may substitute.
• Mobile worker; regular travel within assigned territory and flexibility for evenings/weekends.
• Residence in the assigned territory within Georgia; travel and on-site visits as needed.

Responsibilities

• Assist people with intellectual challenges or developmental disabilities to access and coordinate needed support and services in community settings.
• Coordinate and facilitate gathering information for the support plan and arrange clinically pertinent assessments.
• Review clinical information and recommendations for interventions prior to ISP meetings.
• Assist individuals and families in applying for and maintaining Medicaid eligibility and access community resources.
• Monitor Medicaid redetermination dates and complete required paperwork; maintain eligibility documentation.
• Oversee participation in ISP services and ensure plans meet client and family needs.
• Maintain monthly face-to-face contact and timely documentation; ensure actions tie to the support plan.
• Maintain confidentiality and HIPAA compliance according to policy.
