Job summary

The Columbus Organization is seeking a dedicated case manager to help individuals with intellectual and developmental disabilities access needed services in community settings. You will gather information for ISP plans, assess interventions, and coordinate Medicaid eligibility, with regular field visits and strong regulatory compliance.

The role emphasizes effective communication, program development, and the ability to maintain confidential records while working within DBHDD guidelines.

Qualifications

• Must have a minimum of a bachelor’s degree in a human service field such as social work, psychology, special education, nursing, or related field.
• At least one year of experience serving persons with developmental disabilities in a case management or coordination role, or a Master’s Degree may substitute for the experience.
• This is a mobile position requiring regular travel to different work locations and residence in the assigned territory.

Responsibilities

• Assists people with intellectual challenges or developmental disabilities to access and coordinate needed support and services in community settings.
• Coordinates and facilitates gathering information for the support plan and arranges clinically pertinent assessments.
• Reviews clinical information to identify cost-effective services and supports based on individual needs and preferences.
• Assists individuals and families in applying for/maintaining Medicaid eligibility.
• Maintains monthly face-to-face contact and documents all contacts/services in the support notes.
