Job summary

All Care Home Health seeks an IDD Program Case Manager to coordinate and monitor services for individuals with IDD in Georgia. You will serve as the primary liaison among individuals, families, home managers, and the interdisciplinary team to promote independence and community integration.

The role requires a bachelor's degree in a related field and at least one year of relevant experience, with strong organizational and communication skills, and a valid Georgia driver's license for service area

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Special Education, Nursing, or a related field.
• Minimum of one year of experience with individuals with IDD, behavioral health, social services, or case management.
• Knowledge of person-centered planning principles.
• Strong organizational, communication, and documentation skills.
• Proficiency with electronic health records and Microsoft Office applications.
• Valid Georgia driver's license and reliable transportation.
• Ability to travel within assigned service areas.

Responsibilities

• Coordinate and manage case services for individuals receiving IDD waiver and community-based services.
• Conduct comprehensive assessments and develop person-centered service plans based on individual needs, preferences, and goals.
• Monitor service delivery to ensure quality, compliance, and participant satisfaction.
• Maintain regular contact with individuals served, family members, guardians, and service providers.
• Advocate for individuals' rights, choices, and access to appropriate resources and services.
• Assist individuals in accessing medical, behavioral health, vocational, educational, residential, and community support services.
• Complete required documentation accurately and timely in accordance with Georgia DBHDD, Medicaid, and organizational requirements.
• Monitor health and safety concerns and coordinate interventions as necessary.
• Participate in interdisciplinary team meetings and provide ongoing case updates.
• Ensure compliance with all state, federal, and agency regulations governing IDD services.
• Respond to crisis situations and provide support as needed.
