Qualifications

• Bachelor's degree in a human services field or licensure as a RN with 3 years of relevant experience OR Master's degree with 1 year of relevant experience and RN licensure
• Must have a valid driver’s license
• Must reside in North Carolina within 75 miles of Duplin County
• Must be legally authorized to work in the United States without sponsorship

Responsibilities

• Coordinate and manage care transition activities across settings to support successful discharges and service changes.
• Develop and implement transition plans based on individual needs and transition requirements.
• Collaborate with providers and partners to align services during transition periods.
• Monitor transitions and intervene to resolve delays or barriers.
• Facilitate service connections and ensure continuity following transitions.
